If you think it is normal that there are no misses for 5 minutes, then
simply increase the idle timeout on the server using --idle_tout
option. At 1 req/sec (idle phase is 10% of the peak load), I doubt
having no misses for 5 minutes is normal though.

If you think there should be at least one miss every 5 minutes, then
we will need more information to help you. First of all, does the
no-proxy test work in the same configuration? If yes, check that the
proxy is capable of talking to the servers. Check that the clients are
not receiving error messages from the proxy whenever a miss occurs.
Try requesting a random URL from the simulated server through the
proxy using wget or telnet and see if your request reaches the server.

If you are still stuck, please send me complete client and server-side
console output (compressed with gzip if needed), and we will go from
there.
